Yes it is due to the risk of falling in. However, there are ways to tell if a lake is somewhat safe to skate on.
2. Is it true that if you fall in, the hole you broke through would frozen over?
The water wouldn't freeze that quickly. It'd probably freeze over if it's been a long time.
3. Do you die from the freezing cold water?
You could die of shock from the cold or get hypothermia

I believe he died from cold shock, yes.
